सबसे पहले से हमें जानकारी है कि कोरोना वायरस के सबसे पुराने और सबसे ज्यादा लक्षण हैं, बुखार आना, सूखी खाँसी होना और सांस फूलना | बुखार हमने देखा है कि ज्यादा हाई फीवर नहीं होता, करीब 99-100 एंड 101 के करीब फीवर होता है| खाँसी जो है सूखी होती है | और लक्षण अगर ज्यादा देर चले 4, 5, 7 दिन चले तो उसके बाद फिर सांस फूलता है पेशेंट का |
कोरोना वायरस के और भी लक्षण हैं : ENT और कान, नाक और गले का लक्षण हैं पेशेंट कहता हैं कि हमें कुछ स्मेल नहीं हो रहा या मुंह में टेस्ट नहीं आ रहा जब सांस से स्मेल चला जाता हैं तो हम उसको कहते हैं कि वो कोरोना का सबसे पहले वाला लक्षण है | early symptom of covid-19.
धीरे-धीरे करके जैसे ही ये बीमारी और फ़ैल रही है तो हमें और भी लक्षण पता चल रहे हैं, कमजोरी होना, बदन दर्द होना, सिर में दर्द, loose मोशन होना, पेट में दर्द होना, कभी कभी eye में इन्फेक्शन होना इस तरह के लक्षण भी पाए गए हैं |
कोरोना वायरस ड्रॉपलेट इन्फेक्शन से फैलता है यानी कि अगर किसी ने आपके पास खांसा या उनको छींक आयी तो वो ड्रॉपलेट से वो इन्फेक्शन आपके शरीर में आती है नाक और गले के जरिये और आपके लंग्स या फेफड़े में जाके फैलती है |
इसको फैलने में, वायरल लोड को बढ़ने में, करीब 4-5 दिन लगते हैं| हमने ये देखा है कि कभी- कभी दो हफ्ते तक लग सकते हैं|
जब आपको इन्फेक्शन फैलता है तो सबसे पहला लक्षण-आपको बुखार, खाँसी और धीरे-धीरे कर के सांस फूलना शुरू होता है | ये पहले लक्षण हैं कोरोना वायरस के |
कोरोना वायरस के और भी हमने सीरियस सिम्टम देखे हैं : कुछ हमने देखे हैं बच्चों में जिसको हम cytokine storm कहते हैं जिसमें कि बच्चे के अपने ही टिश्यू अपने ही शरीर के सेल्स को डेस्ट्रॉय करना शुरू कर देते हैं और फिर ICU में ठीक तरीके से उसका इलाज करना पड़ता है |
बाकी लोगों में भी एडल्ट्स एंड ग्रोन up में भी हमने ये देखा है कि कभी-कभी कोरोना वायरस काफी टाइम तक चलता है ये नहीं कि 5-6 दिन | हफ़्तों भर कोरोना वायरस के मरीज उसकी चपेट में रहते हैं और इन लोगों को सांस फूलता है, कमजोरी आती है, बदन दर्द होता है और एक और नया phenomenon हमने देखा है जिसको ब्रेन fog कहते हैं | इससे हमको लगता है कि कोरोना वायरस शायद ब्रेन तक भी फैलता हैं | इन लोगों को मेंटल confusion (state of confusion) होता है और disincentive to work यानि की बिल्कुल भी काम करने की इच्छा नहीं होती|
कोरोना वायरस ज्यादातर हफ्ते 10 दिन में ठीक हो जाता है और पेशेंट asymptomatic हो के अपने काम पर निकल पड़ता है लेकिन हमने देखा है कि कुछ long haul पेशेंटस होते हैं जिनके कोरोना वायरस इन्फेक्शन काफी दिन तक उनको परेशान करते हैं| ये इसलिए करते हैं क्योंकि उनका वायरल लोड ज्यादा होता है और वायरस पूरे शरीर में फ़ैल के हर अंग को या organ को डैमेज करता है और पेशेंट सीरियसली बीमार हो जाता है |
सीरियस कोरोना वायरस पेशेंट के कुछ और लक्षण हमने देखें हैं उसमे दो चीज़ें हैं हार्ट के ऊपर डैमेज और किडनी के ऊपर डैमेज| हार्ट के valves को हार्ट के muscle को कोरोना वायरस डैमेज करके और कभी कभी endocarditis भी करता हैं जिसकी वजह से पेशेंट को परेशानी होती है | कोरोना वायरस के पेशेंट जब ICU में जाते हैं तो हमने ये भी देखा है कि काफी किडनी डैमेज होता है और किडनी डैमेज की वजह से काफी पेशेंट्स को डायलिसिस पर डालना पड़ता है |
कोरोना वायरस के सीरियस पेशेंट पर हमने कुछ और चीज़े देखी हैं उनके हार्ट के ऊपर डैमेज होता है कभी कभी उनकी किडनी के ऊपर डैमेज होता है और ये भी हमने देखा है कि जो पेशेंट ICU में जाते हैं क्योंकि किडनी के ऊपर असर होता है तो काफी पेशेंट को डायलिसिस देना पड़ता है | कोरोना वायरस दिमाग के ऊपर भी असर करता है जिससे पेशेंट को ब्रेन fog होता है यानि कि वो समझने में कोशिश करते हैं कि आस-पास क्या हो रहा है और मेंटल confusion रहता है उनको|
अभी भी कोरोना वायरस से बचने के लिए यहीं तीन उपाय हैं हमारे पास | दूसरे लोगों से 2 मीटर की दूरी मेन्टेन करनी है | हमें बाहर, ख़ास कर जहाँ ज्यादा लोग हों वहां फेस मास्क जिससे नाक और मुंह ढँका रहे वो जरुर करना है और जब भी हम घर वापिस आएं और बीच-बीच में भी जब भी हम बाहर से वापिस आएं, हाथ को कम से कम 20 सेकंड तक साबुन-पानी से धोना है इसी से हम इस इन्फेक्शन से बचे रहेंगे |

Recognizing Day to Day Signs and Symptoms of Coronavirus

DAY TO DAY SYMPTOMS OF COVID-19
Before proceeding, please note that this general overview is compiled for initial self-assessment only and may vary for each individual. If you're not feeling well, you should immediately consult a medical practitioner to have an accurate diagnosis and proper treatment of COVID-19.
The typical daily symptoms are concluded from the study of 138 patients at Zhongnan Hospital of Wuhan University and another study involving 135 patients from Jinyintan Hospital and 56 patients from Wuhan Pulmonary Hospital.
These symptoms are broken down into:
DAY 1 TO DAY 2
The beginning symptoms are similar to the common cold with a mild sore throat and neither having a fever nor feeling tired. Patients can still consume food and drink as usual.
DAY 3
The patient's throats start to feel a bit painful. Body temperature reads at around 36.5° celsius. Although it's uncommon, other symptoms like mild nausea, vomiting or mild diarrhea are possible to set in.
DAY 4
Throat pain becomes more serious. Other symptoms like feeling weak and joint pain start to manifest. The patient may show a temperature reading between 36.5° to 37° celsius.
DAY 5 TO 6
Mild fever starts. The patients show a temperature reading above 37.2° celsius. The second most common symptom, dry cough, also appears. Dyspnea or breathing difficulty may occur occasionally. Most patients in this stage are easily feeling tired. Other symptoms remain about the same. These four symptoms are among the top five key indications of COVID-19 according to the final report of the initial outbreak conducted by the joint mission of China and WHO.
DAY 7
The patients that haven't started recovering by day 7 get more serious coughs and breathing difficulty. Fever can get higher up to 38° celsius. Patients may develop further headache and body pain or worsening diarrhea if there’s any. Many patients are admitted to the hospital at this stage.
DAY 8 TO 9
On the 8th day, the symptoms are likely to be worsened for the patient who has coexisting medical conditions. Severe shortness of breath becomes more frequent. Temperature reading goes well above 38°. In one of the studies, day 9 is the average time when Sepsis starts to affect 40% of the patients.
DAY 10 TO 11
Doctors are ordering imaging tests like chest x-ray to capture the severity of respiratory distress in patients. Patients are having loss of appetite and may be facing abdominal pain. The condition also needs immediate treatment in ICU.
DAY 12 TO 14
For the survivors, the symptoms can be well-managed at this point. Fever tends to get better and breathing difficulties may start to cease on day 13. But Some patients may still be affected by mild cough even after hospital discharge.
DAY 15 TO 16
Day 15 is the opposite condition for the rest of the minority patients . The fragile group must prepare for the possibility of acute cardiac injury or kidney injury.
DAY 17 TO 19
COVID-19 fatality cases happen at around day 18. Before the time, vulnerable patients may develop a secondary infection caused by a new pathogen in the lower respiratory tract. The severe condition may then lead to a blood coagulation and ischemia.
DAY 20 TO 22
The surviving patients are recovered completely from the disease and are discharged from the hospital.

Home Isolation For Patients With Mild Symptoms Of COVID-19

Anytime someone finds out about being tested for Covid-19 they go into a panic frenzy and start fretting about treatment and hospitals not having enough beds and space due to the rapid increase in cases. But not every case of Covid-19 is as serious and you can continue the treatment for Covid-19 at home by following the guidelines for home isolation.

How do you know if your symptoms of Covid-19 are mild or serious? The few things to monitor are like continuous high fever for 5 days, oxygen level below 95, uncontrolled diabetes, above the age of 80, have gone under any serious surgical treatments, etc., if you don’t fall under categories like these, then you can continue your treatment for Covid-19 at home.

If you have mild symptoms of Covid-19, you can quarantine at home by following the guidelines for home isolation. Here are a few types of equipment you’ll need to quarantine for Covid-19 at-home - pulse oximeter, a device to check the blood oxygen levels at home, a thermometer, those who have blood pressure problems and are on medication for the same need a portable BP machine, etc.

The patients quarantining at home need to use these devices to monitor their Covid-19 symptoms regularly. You’re required to check your fever, temperature, and oxygen level every 6 hours. Oxygen level at 95 or above is fine, the oxygen level might vary while you’re testing, to get an accurate count, walk around for a few minutes and then test your oxygen level again, if it stays 95 then the situation is under control. Your temperature might vary from 97-99 but the situation is under control, but if your temperature is 99.5 or above continuously even after taking medication, then it’s an alarming situation and you need to consult the doctor.

Moving on to dietary requirements for home-based care for covid patients. If you’re a diabetic or a blood pressure patient, then it’s essential to keep these problems under control by regular monitoring and following your regular diet to keep your sugar or bp in control. Other than that Covid-19 patients at home can eat the regular food that is being made for everyone else and there are no special diet restrictions. Since patients lose their taste due to fever they might not be able to eat as much, however, you have to maintain your liquid intake to a sufficient level of 2.5-7 liter water every day. Other than that doctors recommend a high protein diet to maintain the nutritional level for at least the initial 10-14 days.

The next important activity for home-based care for covid patients is breathing exercises. Only medication will not be effective and Covid-19 patients at home need to do breathing exercises, if you know pranayama, do that, if not normal deep breathing exercises are good too. The next very important activity is awake proning, or lying on your stomach. A lot of people don’t take it seriously but doctors highly recommend it. Awake proning for 6 hours in a day is very important for treatment of Covid-19 at home and maintaining oxygen levels.

Maintaining the above-mentioned parameters and regular follow-ups with doctors are the guidelines for home isolation for the treatment of people with mild symptoms of Covid-19 at home.

Patients with COVID-19 may experience happy hypoxia or silent hypoxia. This is the most important symptom, and so it is advised to keep checking your oxygen level on a pulse oximeter.
At the beginning, the patient might have flu-like symptoms such as mild cough, throat pain, or sore throat, and a fever may develop. What we have observed with COVID-19 is that the fever lasts for 2 to 3 days. After that, it subsides. However, a patient may continue to experience a mild dry cough or an irritation or discomfort in the throat.
On the 6th or 7th day from the onset of symptoms, some patients may develop respiratory problems, and might experience breathlessness while walking, while sitting or while climbing stairs. This is the most important symptom. So, it is said that keep checking your oxygen level with an oximeter because happy hypoxia or silent hypoxia may occur.

Patients who require ICU care and start to experience shortness of breath are those who generally have an impact on their lungs.

There are 5 -10 % of patients whose brain is affected due to corona or COVID-19. We also see that some patients experience acute kidney injury.
However, all these happen in negligible proportions in comparison to that of the lungs.

The most troublesome symptom that can happen in 90% of cases is breathlessness. However, shortness of breath doesn't need to happen from the very first day. Sometimes, even 5 to 6 days after the fever has subsided, the patient may experience breathlessness.

If these symptoms persist, please consult your doctor or physician.

COVID-19 में Happy hypoxia या silent hypoxia होता है |ये सबसे important symptom है और इसीलिए कहा गया है की pulse oximeter पर अपनी ऑक्सीजन level check करते रहें|

देखिये. शुरुआत में पेशेंट को flu जैसे symptoms हो सकते हैं| हल्की खांसी, गले में दर्द, या खराश, और उसके साथ में बुखार की शिकायत develop होती है|

Covid में जो normally हम देख रहे हैं कि दो से तीन दिन का बुखार होता है, उसके बाद ज्वार या बुख़ारी settle हो जाता है, ठीक हो जाता है, पेशेंट को हल्की सुखी ख़ासी रहती है और गले में irritation या परेशानी रहती है, खराश बनी रहती है|

but symptom onset से छठे-सातवे दिन, कुछ मरीजों में सांस की परेशानी develop होती है, और चलने में सांस फूलने लगता है या कभी-कभी बैठे-बैठे भी उनकी सांस फूलने लगती है, specially patient note करता है कि सीढ़िया चढ़ने में, stair climbing में उनको परेशानी आ रही है, सांस फूलने लगी है| और ये symptom अगर progress कर रहे हैं या persist कर रहे हैं तो वो time है जब आपको अपने डॉक्टर से या चिकित्सक से सलाह लेनी है. जिस particular अंग के बारे में बात करे की सबसे ज्यादा effect जो आ रहा है वो lungs पर आ रहा है तो जितने serious patients हैं जिनको ICU की ज़रूरत पड़ती है जिनकी सांस फूलने लगती है ये सारे patients वो है जिनका प्रायः तौर पर lungs पर या फेफड़ों पर असर होता है|

इसके अलावा 5 to 10 percent patients ऐसे हैं जिसमे कोरोना की वजह से या covid 19 की वजह से दिमाग पर असर होता है| कुछ patients में किडनी पर acute किडनी injury हो सकती है but ये सब कुछ lungs के comparison में, फेफड़ों के comparison में बहुत ही कम मात्रा में होता है, बहुत ही कम अनुपात में देखा गया है| देखिये 90 percent cases में सबसे worrisome case या symptom जो हो सकता है वो है breathlessness या सांस का फूलना|ज़रूरी नहीं है की सांस का फूलना पहले ही दिन से हो|

जैसा की मैंने पहले भी बताया ज्वर या बुखार के चले जाने के बाद भी 5 से 6 दिन के बाद इस तरह की complain, इस की शिकायत develop हो सकती है और पेशेंट की सांस फूलना बाद में प्रकट होता है, या बाद में patient महसूस करता है|ये सबसे important symptom है इसीलिए कहा गया है oximeter पर अपना oxygen level check करते रहे क्योंकि particularly corona disease में happy hypoxia या silent hypoxia होता है|

अगर इसको सही अर्थों में समझना चाहे तो happy hypoxia का मतलब है की मरीज़ के शरीर में ऑक्सीजन की मात्रा कम हो रही है पर मरीज़ के शरीर को, मरीज़ के मस्तिष्क को इसका आभास नहीं होता|

Patients come to us with flu symptoms and most of them have a panic situation that they have acquired COVID-19 disease. See, COVID-19 generally has a high grade of fever, point number 1

Point number 2, they have sore throat. One more thing is, they present with dry cough

And patients start to have difficulty in breathing
The fever is not subsiding even after three days
And the patient continues to have breathing difficulties
The definitely he has to consult a local physician


In normal flu, fever is generally in milder form
They have simple sore throat which subsides in three days on its own

Fever is a very simple term, but it has so much in it
There are fevers that do not have flu as well
Like, an episode of three to four days of fever
following which there is body pain, joints pain and all that

So in that case it might be a dengue fever or malarial fever
Malarial fevers generally present with fever with chills and all that
which occurs every two days

If a fever is not subsiding for three days then have to generally contact a physician


Fever is generally due to an infectious cause and an non infectious cause
The most common are infectious cause
Infectious fevers again the causes are multiple
Like bacterial infections are more common and viral fevers are more common
There are fungal infections, there will be parasitic infections as well
Like Malaria, Helmentia

Non infectious causes of fever are also plenty
All the cancers like Lymphoma, Leukemia, they present with persistent fevers running for long days
Drugs like Atropine can cause fever
In heat stroke too the patient might have a fever
which is a non infectious cause

So the reasons for fever are plenty
So we have to find out what are the causes of fever
for that you need to consult a physician

You cannot prevent most of the fevers
Few fevers like malaria and dengue, vector borne fevers can be controlled by effectively maintaining the hygiene around the environment

Cover19 disease, in which individual hygiene matters a lot
Maintaining hygiene of your hands, maintaining social distancing
and using masks in crowded places.


If the patient has fever with flu
I request them to just isolate themselves to a single room
And if the patient has any other symptoms aggravating like
high grade of fever is still persistent
or breathing difficulty or something like that is there
for that you need to consult a physician
